Colonial House Inpatient Facility located at 1300 Woodberry Road in York, PA. The facility was built expressly for our purpose and occupies 2½ acres of York County, PA countryside. Spacious recreation and dining areas, functional therapy rooms, administrative offices, as well as second floor residential facilities are all located in one building for maximum convenience, safety and security. The pleasantly decorated residence floor includes comfortable bedrooms, separate reading areas, and ample shower facilities. With two and a half acres of land surrounding the building, there is a beautiful view from every window. Our inpatient facility provides the calm and serenity needed for person’s journey to recovery.
Colonial House Inpatient is a licensed residential treatment program for adult men and women with substance use disorders and co-occurring medical, mental health, or behavioral conditions.
Our program provides 24/7 medically monitored, high-intensity residential care (ASAM 3.5) in a safe, structured, and supportive environment. Treatment is based on a habilitative model, focusing on building life skills, promoting prosocial behaviors, and supporting long-term recovery.
Length of stay is individualized and based on clinical progress and treatment goals.
We offer integrated, whole-person care that addresses substance use, mental health, and medical needs simultaneously, with an emphasis on cognitive restructuring, communication and interpersonal skills, trigger identification and management, and resiliency and trauma recovery

- HIV/HCV testing, treatment and post treatment follow up

Admissions Criteria:
- Age 18+
- Substance use disorder diagnosis
- Co-occurring mental health diagnosis
- Medically stable (no acute withdrawal symptoms)
- Meets ASAM 3.5 level of care

Colonial House Outpatient Facility, is conveniently located at 1 East Market Street, Suite 202 in historic downtown York, Pennsylvania. The program is accessible via public transportation and is within walking distance from numerous community partners.
Colonial House Outpatient provides flexible, community-based treatment for adults with substance use disorders and co-occurring conditions.
We offer multiple levels of care aligned with ASAM criteria, allowing individuals to receive the support they need while maintaining responsibilities at home, work, or school.
Levels of Care
We offer a full continuum of outpatient services:
Partial Hospitalization Program (PHP) – ASAM 2.5
- Minimum 20 hours/week
- Structured daily programming (daily groups, individual sessions 2x weekly)
- Ideal for individuals needing intensive support without residential care
Intensive Outpatient Program (IOP) – ASAM 2.1
- 9–19 hours/week (groups 3x week, individual session weekly)
- Structured therapy with flexibility
Outpatient Program (OP) – ASAM 1.0
- Less than 9 hours/week (group 1x weekly and/or individual sessions)
- Ongoing support and relapse prevention
Services Include:

- Medications for Substance Use Disorder
- Mental health and substance use education
- DUI Highway Safety Classes
- Vocational and educational support
- Referrals for medical, psychiatric, and social services
